To process a claim, the following certified documents must be submitted:
Death Certificate
Identity Document of the Deceased
BI-63 Form
Claimant’s Identity Document
If the death was accidental, a Police Report will also be required.

Once the documents have been received, the claim will be reviewed to confirm:

Once the claim has been approved, the funeral benefit payment will be processed.
